Submitting a Referral Through the Health Plan Sharing Portal

This article explains how health plans can submit a referral for a new or existing pregnancy through the Health Plan Sharing Portal in the Well Family System.

Steps to Submit a Referral 

Step 1: Log into WFS

Step 2: Search for Client Record 

Search by Medicaid ID and other available search options.

Step 3: If No Record Found 

Refer as a New Case.

Step 4: If Record Found 


Verify if this is a New Pregnancy or an Existing Pregnancy. 

 

Step 5: Based on Pregnancy Status 

  • New Pregnancy: Complete a New Referral.

  • Existing Pregnancy and Record Closed: Submit a Request to Reopen. 

Step 6: Submit Referral 
Outcome 

  • Referral appears in the Referral Tickler on the Main Dashboard. 

  • Referral is routed to: 

  • CIR Incoming Cases 

  • Cases from Health Plans 

  • Requests to Reopen
